| How to ship cases to us |
| 1. Dentists please prepare the following items before sending the case or a digital case: |

| a. Write to us a worksheet, fill it with as much detailed information as possible; such as patient names, patient age, tooth numbers, service items ordered, single crowns or bridge, return dates, designs, color, special comments etc. |
| b. Take working impression (model) and opposite impression (model), study impression (model) if necessary, pay attention to tissue are which is important to dentures. |
| c. Bite registration (if necessary) |
| d. Please include all implant parts if the case is an implant, including lab parts and let us know what type of implant is prefered. |
| e. Prepare airway bill for shipping (you can choose whichever shipping courier from the following, such as DHL, UPS or Fedex), You may also need to prepare two commercial invoices for Customs and a non wooden declaration paper. Please contact our service person, we have those format you probably can use. |
